Maryland Vaccines for Children Program

VFC Provider Enrollment

Per federal CDC regulations, VFC providers are required to re-enroll annually in order to receive VFC vaccine. The 2024 VFC provider re-enrollment process will be done through an online survey.

Recommended steps to complete the 2024 VFC online enrollment survey:

  1. Print a “sample copy” of the survey and the “Suggested Imm. Schedule using Combination Vaccines” to review with the appropriate medical provider and office staff before beginning the official online survey. (Do NOT mail or fax the “sample copy” of the survey. It will not be accepted.)
    • Please review and consider your vaccine brand preferences carefully. Selected vaccine brand preferences will remain in effect for all of 2024. Requests to change selected vaccine preferences will require a written justification to the Maryland VFC Program.
  2. Enter the “sample copy” data into the online survey, by clicking on the blue “online survey” button located to the right. This will take you to the ImmuNet website. Plan to spend at least 15 – 20 minutes completing the online survey.
  3. Log into ImmuNet.  Click on “VFC Inventory/Orders”.  On the right side of the page, click on “VFC Enrollment Survey”.  Complete the survey using the “sample copy” data.
  4. Complete one 2024 VFC online enrollment survey for each VFC PIN. 

You will receive verification in January from the Maryland VFC program that your survey information has been received and approved.
